Youth Shelter Case ManagerSchedule: Full time, benefits-eligible! Monday through Friday 8am-5pm but will require flexibility based on client and program needs.
Pay: $18 / hr
Benefits - Approximate full-time benefits package value = $21,000+

Volunteers of America, Utah provides community-supported paths for those who are vulnerable to improve their lives and increase their self-reliance.
You will be based out of the Youth Resource Center; a refuge for youth ages 15-22 experiencing homelessness, mental health conditions, and extreme disconnect from positive adult role models and who benefit greatly from the consistent support of healthy adults.This position will conduct motivational interviews with clients to evaluate their housing focused needs and goals. Work closely with Diversion to make sure that Vital Docs have been secured and maintained in order to move into housing. Make client focused referrals to housing services, both internally and externally. Work directly with client to help determine their best course of action while connecting to community partners.
- Maintain a caseload of up to 20 shelter clients.
- Assist clients in developing goals/service plan that will help them overcome homelessness, identify and address barriers to housing.
- Meet/check-in with case managed clients at least once a week (All clients should have at least one case note entered Bi-weekly).
- Frequency of meetings with clients on caseload expectation of at least once a week.
- case note requirements (all clients should have at least 1 case note/week. 7200-7800 minutes/month case note requirement)
- Have and attend daily office hours on shelter floor, 2 hours a day, per shift.
- Help clients secure housing by supporting them with each step of the process (Fill out application, request app fee, turn in application to property manager, and repeat if needed).
- Help clients set and make appointments with potential housing options
- Participate in regularly scheduled clinical case review to ensure appropriate case/service planning, and referral of clients to the most appropriate service provider.
- Provide direct, Trauma Informed Care case management services to clients, including crisis intervention, outreach, ongoing assessments (i.e. SPDAT, Lethality Assessment etc.), goal setting, creative problem solving, agency and community resource utilization, and network building.
- Identify priority needs, (i.e. pregnant, IV/IM drug user, dual diagnosis, and other medical conditions) and coordinate services with diversion as needed.
- Attend weekly staff/clinical staffing meetings and other meetings as assigned.
- Assist in resolving conflicts utilizing de-escalation skills and trauma informed practices. Also provides immediate and professional response to any security /safety emergency or disturbance within or immediately outside facility following appropriate policies and procedures.
- Keep records, case management notes, and statistical data as required and participate in research and/or special projects as needed.
- A degree in the field of social services and two years’ experience working in the social services field preferred.
- Must be able to qualify and pass Utah Department of Human Services Case Management Certification within 60 days of being hired.
- Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public.
- Ability to utilize Trauma Informed practices to act and make sound decisions within the scope of position responsibilities as well as reach out for supervision for major situations.
- Willingness to accept supervision and direction.
- Ability to move around the facility and interact with clients for extended periods.
- Familiarity with Trauma Informed Care, Motivational Interviewing and Housing First philosophies and willingness to incorporate them in your daily work.
- Must be at least 21 years of age, possess a current driver’s license, have a good driving record and be insurable on the agency liability policy.
- Ability to lift and carry 50 lbs.
