Schedule: Full Time - 40 hrs/week - benefits eligible!
Pay Rate: $17 / hour

The Homeless Outreach program is a dynamic and unique team. Our staff work autonomously out in the community to assist clients with basic needs, case management and referrals to higher levels of care or community partners. Objectives will be to create supportive relationships with homeless individuals through street outreach and referrals to community partners to prioritize meeting clients where they are at.

This position provides outreach services to Salt Lake County’s unsheltered individuals and/or families in order to connect clients with supportive services with the goal to decrease Salt Lake County’s unsheltered population and connect individuals with safe and stable housing. Our county care coordination team is made up of two individuals. This team will be provided a work vehicle and will be expected to provide services to unsheltered clients across the county limit. Daily tasks may include providing clients with essential supplies, driving clients to community partner appointments, and assisting clients with obtaining vital documents that will later be essential in the housing process. Care coordinators will have the ability to build rapport with clients to assess housing motivation and add clients to HOP’s internal case management referral list. This team is expected to respond and assess community referrals. In this position you will also respond to community concerns, assist with community education, and problem solve around homelessness issues. 

Essential Duties

  • Proactively engages and initiates contact with clients in accessing basic needs items and community-based resources, to help them overcome homelessness.
  • Prioritizes client engagement in Salt Lake County, identifying priority areas, coordinate needs, and address barriers.
  • Identify priority needs, (i.e. pregnant, IV/IM drug user, dual diagnosis, and other medical conditions) and refer/coordinate services as needed.
  • Provide direct, Trauma Informed Care to clients experiencing unsheltered homelessness, including crisis intervention, outreach, ongoing assessments (i.e. VI-SPDAT, SPDAT, Lethality Assessment etc.), creative problem solving, agency and community resource utilization, and network building.
  • Assess medical emergencies, and provides immediate and professional response to any emergency following appropriate policies and procedures. (contacting emergency medical, utilize Naloxone kits, Etc.)
  • Participate in regularly scheduled staff meetings in order to ensure appropriate case planning and referral of clients to the most appropriate service provider.
  •  Ensuring that all paperwork related to client and program activities is properly completed, managed confidentially and stored in an appropriate location and submitted in a timely manner.
  • Assist in resolving conflicts utilizing de-escalation skills and trauma informed practices.
  • Evaluate own performance to increase effectiveness as an Care Coordinator, and maintains willingness to accept supervision and direction.
  • Maintain positive, professional interactions co-workers, and all other community providers including; Homeless Resource Center staff, emergency services, fire, police and sheriff, criminal justice, staff from other community providers, neighborhood residents and businesses, client family members, agency donors, community support groups and referral sources.
  • Manage purchase receipts and submit them in a timely manner.
  • Assist with community education, response, and problem solving around homelessness issues.
  • Works to Identify and refer clients to Case Management when appropriate. Ability to utilize Trauma Informed practices to act and make sound decisions within the scope of position responsibilities as well as reach out for supervision for major situations.
  • Attend work on a punctual, regular, and predictable basis.

Qualifications
  • Ability to learn and utilize software programs in use by VOA.
  • Ability to establish and maintain appropriate, professional boundaries with clients and coworkers.
  • Ability to prioritize multiple tasks; flexibility to respond to changes.
  • Ability to drive company vehicle; possess a current driver’s license and have a good driving record.
  • Bachelor's degree in a related field OR one year of work experience OR a combination of education and experience.
  • Ability to work with clients regardless of age, ethnicity, socioeconomic status, and/or sexual orientation, etc.
  • Ability to work in a team environment.
  • Ability to perform outreach in extreme weather conditions, including summer heat and winter snow; be willing and able to perform outreach on foot; walking, hiking, and navigating various buildings, campsites, and environments and interact with clients for extended periods.
  •  Must be able to lift 25 Ibs.
  • Must be at least 21 years of age, possess a current driver’s license, have a good driving record and be insurable on the agency liability policy.
  • Familiarity with Trauma Informed Care, Motivational Interviewing and Housing First philosophies and willingness to incorporate them in your daily work.
