Case Manager - COVID 19 Care

Schedule: Full Time -- benefits-eligible!
Pay: $17 per hour

Volunteers of America, Utah builds a community supported bridge that provides a path for those who are vulnerable to improve their lives and increase their self-reliance.

The Housing and Benefits Case Manager will link individuals experiencing homelessness in our detoxification and withdrawal management programs to housing and benefit opportunities and resources in our community with the goal of assisting them to achieve stable housing and critical benefits in a trauma informed way.

Essential Duties

  • Manages Ongoing COVID 19 Care and Safety
    1. Creates a daily line list of the clients who have received a rapid test at the door of Detox (on Monday he will complete the line list for the weekend).
    2. Weekly rapid testing of a sampling of clients or all of the clients depending on the level of COVID exposure in the facility who have been in the facility 5 days or longer.
    3. When on shift, when we have a client test positive, the CCCM will arrange for the client to be transferred to the QI site:
  • Put client in the Intake office or isolate client in the small dorm whichever is available
  • Clean or arrange to have the client’s bed cleaned and his belongings packed
  • Contact the QI site, let the client talk to the triage nurse, and arrange for the room
  • Arrange for the client to have all his necessary medications while in QI
  • Transport client to the QI
  • Case Manages a small Caseload of Clients
  • Clients who may need assistance while residing at the QI site.
  • Clients who are medically compromised and may need individualized care.
  • Clients who are working with the ACT team.
  • Client Services:
  • Provide case management services to clients and document all services provided.
  • CCCM must engage with clients in an appropriate fashion; following all VOA Boundaries as outlined in the Detox Services Policies and Procedures Manual.
  • Manage difficult client behaviors by encouraging adherence to client expectations.
  • Perform Case Management Assessments with clients within 5 business days of clients’ admission to Detox.
  • Identify special needs (dual diagnosis, chronic medical conditions, other conditions needing extra attention, etc.) and coordinate services as needed.
  • Utilize the referral process by scheduling assessments through ARS or other specific treatment facility protocols.
  • When appropriate, work with clients’ families to move clients into self-sufficiency.
  • Input client case notes into UWITS in a timely manner:
  • Case notes should accurately tell a clients’ history, drug usage, withdrawal management, medical, mental health, recovery engagement, legal involvement, case management action on behalf of client, etc. Painting an accurate picture of this clients’ current episode in conjunction with past episodes in VOA Detox Services, including the client’s current plan for this episode.
  • Crisis Intervention
  • Assess medical emergencies, provide CPR and First Aid, and coordinate medical services as needed.
  • Utilize de-escalation skills to manage client crisis by adhering to the De-escalation Training.
  • Maintain positive, professional interactions with community partners including, emergency services, fire, law enforcement, criminal justice, and staff from other community providers, client family members, agency donors, community support groups and referral sources.
  • Learn, utilize and endorse agency selected clinical techniques, including Motivational Interviewing and Trauma Informed practices.
  • Participate and present client information at case reviews with other case managers, peer support, Program Director, and Clinical Supervisor in Clinical Supervision once a week.
  • Participate in Management On-Call Schedule
  • Maintain appropriate, professional boundaries with clients, volunteers and co-workers.
  • Attends work on a regular and predictable basis.
  • Adhere to Volunteers of America, Utah personnel and program policies and procedures.
  • Manages Fourth Street Mobile Medical Clinic visits to Detox
  • It is the duty of this staff member to coordinate client care with the Mobile Clinic.
  • That will include working with the case managers and clients to schedule those clients most in need of an appointment and then schedule those who are interested in having an appointment.
  • When the Mobile Clinic is here the duties will include:
  • Walking clients to the Van
  • Talking with Mobile staff about each client’s follow up care and relaying this information to the clients’ case manager.
  • Maintain walkie-talkie contact for safety with Mobile staff while assisting our clients.
  • Manages Client Vaccinations and Flu Shots
    1. We are keeping track of clients who are vaccinated at our facility to the best of our ability.
    2. Taking Clients to get vaccinated at Fourth Street Clinic when we have clients who are interested.
    3. Arranges with 4th Street Clinic Vaccination and Flu shot events.
    4. Assists the Salt Lake County Health Department with all vaccination events at Detox.
      • Giving out incentives to clients; documenting gift cards as they are given to clients.
      • Recording all vaccinations in the Men’s Detox vaccination file
  • Conduct Client Groups
    1. Participates in the Orientation schedule
  •  

  • Contribute to an environment of mutual trust and respect by treating others fairly, demonstrating empathy, and effectively managing interpersonal conflict to preserve constructive, professional relationships.
  • Have a clear understanding of and adherence to Federal Confidentiality and HIPAA laws.
  • Obtain and maintain CPR and First Aid certification.
  • Proactively expand and integrate knowledge of community resources and maintain familiarity with issues and trends in the alcohol/drug treatment field.
  • Take action and make decisions within position’s scope of responsibility.
  • Demonstrate perseverance and stamina in the delivery of services to clients with complex problems.
  • Have the ability to attend meetings at multiple locations.
  • Participate in various meetings and trainings as needed.
  • Performs other functions as necessary or assigned.

Qualifications
  • Understanding and/or training in:
    • Trauma Informed Approach
    • Motivational Interviewing
    • Stages of Change
  • Social Services degree, 2 years working in substance abuse treatment services, or an adequate combination.
  • Ability to interact in a professional manner with a diverse workforce, clients, and the public.
  • Excellent verbal and written communication skills; attention to detail.
  • Maintain confidentiality of information and appropriate, professional boundaries with clients, volunteers, and co-workers.
  • Remains calm and responds appropriately in crises.
  • Ability to prioritize multiple tasks; flexibility to respond to changes.
  • Organize, prioritize and independently complete multiple tasks.
  • Punctual and dependable attendance.
  • Must be at least 21 years of age, possess a current driver’s license and have a good driving record.
  • Must be able to walk up and down stairs.
