Volunteers of America, Utah builds a community-supported bridge that provides a path for those who are vulnerable to improve their lives and increase their self-reliance.

Volunteers of America, Utah's Homeless Outreach Program provides basic needs services and connects individuals experiencing homelessness in Salt Lake County to housing, legal, and medical services. Materials provided include food, water, hygiene supplies, clothes, shoes, sunscreen, bug spray, hand warmers, sleeping bags, and other supplies based on availability. The Homeless Outreach Program's Housing Case Manager will provide consistent, individualized, and thorough case management services to formerly homeless clients in a time-limited scattered site housing model.

Our Expectations

  • Attend work on a punctual, regular, and predictable basis.
  • Maintain client confidentiality as outlined in the training module on confidentiality.
  • Maintain positive, professional interactions with community resources including: Homeless Resource Center staff, emergency services, fire, police and sheriff, criminal justice, staff from other community providers, neighborhood residents and businesses, client family members, agency donors, community support groups and referral sources.

Essential Duties

  • Assist clients with housing searches and property management engagement.
  • Provide case management services to clients to help them maintain housing and address other needs.
  • Participate in regularly scheduled clinical case reviews in order to ensure appropriate case planning and referral of clients to the most appropriate service provider.
  • Develop and/or maintain client case file, as directed.
  • Complete all client progress notes as outlined in the training module on Progress Notes.
  • Develop positive relationship with referral resources.
  • Work with clients and other community partners to move clients toward housing stability.
  • Assess client needs utilizing appropriate agency-approved assessment tools.
  • Attend work on a punctual, regular, and predictable basis.
Qualifications
  • One year case management experience.
  • Valid Utah driver’s license with clean driving record.
  • Work effectively and ethically with a diverse group of people.
  • Maintains appropriate, professional boundaries with clients and coworkers.
  • Exhibits clear communications skills and cooperates with others.
  • Takes action and makes decisions within the scope of position responsibilities.
  • Considers alternatives and consequences before making independent decisions.
  • Remain calm and responds appropriately in crises.
  • Must be at least 21 years of age, have a good driving record, and be insurable on the agency liability policy.
