Medical Assistant / Care Coordinator - Treatment Services

Schedule: Full time -  Benefits eligible!
Pay: $18 / hr for CNA, $20 / hr for Certified Medical Assistant

Full Time Benefits - Approximate full-time benefits package value = $21,000+:

Position Summary
Employee will coordinate with the residential clients on scheduling and follow up on appointments. This position will work closely with the medical team to ensure client’s needs are met.


Essential Duties

  • Ensure clients have prescribed medications on hand. Pick up client medications while following VOA policy.
  • Coordinate scheduling and follow up medical appointments with patients for internal and external providers. Ensure the client is aware of appointments when needed.
  • Communicate with VOA, clinics, hospitals, medical/dental practices, and pharmacies to coordinate client medical care for medications, records and referrals.
  • Triage medical situations raised by clients and residential staff and escalate to RN and/or clinic.
  • Act as a liaison between programs and providers, including medication changes and behavioral issues.
  • Contact insurance to coordinate covered services, care, and co-payments.
  • Maintain proper patient medication storage.
  • Maintain facility emergency kits with adequate medical supplies.
  • Participate in med pass. Train staff on how to pass medications.
  • Audit medication storage areas to ensure only current client medications are being stored.
  • Work with staff manager bi-weekly to dispose of medications left behind or that need to be discarded according to VOA policy.
  • Train staff on medication policy and procedures.
  • Audit medication log for accuracy and report findings to the RN/Program Manager.
  • Record keeping. Ensure all paperwork related to client medications is completed properly, managed confidentially, and stored in the appropriate location.
  • Maintain a spreadsheet of clients connected with Librium or other MAT.
  • Ensure daily NARC counts are completed and reported at each shift change. Report discrepancies to Program Manager/RN.
  • Ensure an incident report on all emergency or critical incidents is complete by the end of shift.
  • Attend work as scheduled and arrive at the designated time.  
  • Work must be performed on-site due to the need to provide direct client care.

 

Secondary Duties

  • Maintain positive, professional interactions with community resources including, emergency services, fire, police and sheriff, criminal justice, staff from other community providers, client family members, agency donors, community support groups and referral sources.
  • Attend daily team meetings and weekly treatment teams. Attending all other staff meetings and training courses as assigned.
  • Transport clients to appointments, if necessary, as per VOA policy.
  • Utilize de-escalation skills to manage client crisis by adhering to the De-escalation Training. Utilize supervisory support as needed.
  • Follow all licensing and health department rules and regulations.
  • Perform other functions as necessary or assigned.

Qualifications

High school diploma or equivalent preferred and (2 years of social services or homelessness-related experience) or any combination of education, experience, and training which provides the following knowledge, skills, and abilities:

  • CNA or Medical Assistant Certificate Preferred.
  • A high school diploma or GED, associate degree and one year experience in the medical field preferred. A combination of education and experience will be considered.
  • Exhibits clear communication skills and has a clear understanding of medication policies.
  • Maintain appropriate, professional boundaries with clients and co-workers.
  • Remains calm and responds appropriately in crisis.
  • Organizes, prioritizes, and accomplishes multiple tasks. Attention to detail.
  • Computer literate with proficiency in Microsoft Office.
  • Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public.
  • Willingness to accept supervision and direction.
  • Become CPR and First Aid Certified.
  • If driving is a part of the job, must be at least 21 years of age, possess a current driver’s license, have a good driving record, and be insurable on the agency liability policy.
  • Must be able to pass Utah-DHS – Office of Licensing background screening and pre-employment drug screen.

 

Physical Demands 

  • Ability to lift and carry 25 lbs.
  • Ability to walk up and down stairs.
  • Ability to move around the facility, interacting with clients for extended periods.
  • Work is generally performed in an office environment.  May entail using a computer for extended periods of time.
