Description
The Peer Care Coordinator is a part of a small team whose focus is to carry out the grant-funded mission to integrate behavioral health and primary care. The individual will work closely with clients to assist them in receiving quality health care, motivating them to set and achieve treatment goals, and linking them to appropriate services.
 
Essential Duties
1.    Arrange and conduct prevention and health promotion Evidence Based Practices.
2.    Connect clients with peer supports including self-help and advocacy groups.
3.    Maintain close relations and collaborate with the on-site medical clinic to coordinate care for clients.
4.    Assist clients with symptom monitoring and management skills to minimize negative effects of chronic conditions in daily living.
5.    Provide active outreach as necessary, including crisis intervention and management, and acquisition of appropriate community supports.
6.    Perform assessments at baseline and in 6-month intervals for medical and mental health data.
7.    Participate in case consultation/staffings.
 
Qualifications

Bachelor’s degree in social work or a related field or any combination of education, experience, and training which provides the following knowledge, skills and abilities:
1.    Certified Peer Support Specialist training completed or in progress. 
2.    Personal experience with substance use disorder recovery and/or mental illness management.
3.    Previous training and experience working with severe and persistent mental illness and substance use disorders.
4.    Ability to be proactive, initiate daily tasks, and work as a team member.
5.    Basic computer skills including Microsoft Word and Excel spread sheets.
6.    Knowledge of nursing and medical field preferred but not required.
7.    Ability to enter consumers’ dwellings, including climbing stairs.
8.    Utah Driver’s License and good driving record.
9.    Pass a criminal background check and drug screen.


$17.00-$19.50/hour
