Description
Provide direct and indirect clinical services to clients utilizing Trauma Informed practices. The PICC Line Program is a partnership with Intermountain Healthcare to transition clients from the hospital to one of our social model detox facilities to complete their IV antibiotics and be connected to needed resources and services. Direct service includes: assessment, service planning, individual, group counseling. Indirect service includes: supervision, staff meetings, and all paperwork associated with client care.

Volunteers of America, Utah provides a bridge to self-reliance and health for vulnerable individuals and populations who struggle with homelessness, addiction and mental illness in our Wasatch Front communities.

Essential Duties
  • Provide clinical case management services to clients in the PICC Line program and additional clients as assigned.
  • Attend weekly client staffing meetings with Intermountain Healthcare. Meet with clients in the hospital prior to discharge to build rapport and prepare for transition to VOA.
  • Create and maintain positive working relations with Intermountain Healthcare, Home Health, Project Reality and other service providers. Work in collaboration with community partners to meet the needs of the clients and coordinate appropriate care.
  • Work with clients to establish a care and coordination plan while in the program and a discharge plan that could include assessment, treatment planning, individual counseling, referral to appropriate treatment facilities, employment services, housing opportunities and/or other community resources.
  • Facilitate Weekly Clinical Staffing with Detox and Center for Women & Children case managers. 
  • Transport clients to and from hospital and other appointments as needed using an agency vehicle.
  • Provide weekly Seeking Safety and/or psycho educational groups for clients.
  • Participate in clinical staffing weekly and decision making for Detox and Center for Women and Children.
  • Document all services provided to clients in a timely manner.
  • Contribute to an environment of mutual trust and respect by treating others fairly, demonstrating empathy, and effectively managing interpersonal conflict to preserve constructive, professional relationships.
  • Participate in Management On-Call Schedule
Qualifications

  • Possess a Master's degree in social work or clinical mental health. CSW or ACMHC preferred.
  • Applicant must be able to qualify and complete Adult Case Management Certification with the Department of Human Services within 30 days of hire.
  • Maintain appropriate, professional boundaries with clients, volunteers and co-workers.
  • Ability to utilize all the tools provided that would assist in your work (i.e. phone, computer, fax, etc.)
  • Ability to interact in a professional manner with a diverse workforce, clients, and the public.
  • Understand and adhere to Federal Confidentiality and HIPAA laws.
  • Must be able to walk up and down stairs.
  • Must be at least 21 years of age, possess a current driver’s license and have a good driving record.
  • Pass a criminal background check and a drug screen.
Full-time, M - F days
Salary: $43,680 - $45,000

    Medical insurance:    $6.09 per paycheck for individual coverage 
    Dental insurance:    $1.71 per paycheck for individual coverage
    Vision insurance    $2.96 per paycheck for individual coverage
    Health savings account:    VOA contributes $300 per quarter to the employee's account; employee contributes $25.96 per pay check
    Life insurance:    $50,000 for employee; $2,000 for each dependent
    Paid holidays:    11 days per year
    Vacation:            80 hours accrues the first year and increases
    Sick time:            96 hours accrues each year
    403b Retirement plan:    Employer match after 1 year
