The Geraldine E. King Women's Resource Center exists to meet the distinctive needs of women who are experiencing homelessness by providing shelter, case management, life skills training, and connection to community services and resources. The ID & Medical Coordinator will work within the Geraldine E. King Women’s Resource Center (WRC) to coordinate vaccination and testing efforts and medical coordination with medical providers for those who are currently experiencing homelessness. This will include working closely with local health care providers, organizing pop-up vaccinations/testing events, overseeing data collection and documentation, completing grant reports, etc.
The center recognizes and meets the needs of those at risk and homeless populations including: single women; domestic violence victims; individuals with behavioral health disorders; individuals who are medically frail/terminally ill [and/or] those exiting prison or jail. All activities are undertaken with the ultimate goal of assisting all clients in obtaining housing.
- Oversees vaccination, testing, and medical events related to infectious diseases within the WRC. This role will take point on the planning and implementation of the events in coordination with the HRC staff and local health care providers.
- Prepare and maintain required Infectious disease vaccination/testing tracking; Oversee data entry for all vaccination/testing data and prepare and send out monthly reports.
- Works collaboratively with community health care workers, ID specialists, local health care providers, and the case management teams for any follow-up care needed.
- Assists clients in accessing health-related services, including but not limited to referrals to skilled nursing facilities in coordination with the HRC and community health care staff.
- Create an efficient way to attain and report out on client feedback of services.
- Assess and report unmet needs and barriers to vaccinating, testing, and ID mitigation for those who are experiencing homelessness.
- Provides scientifically accurate information about infectious diseases as needed related to vaccines and infectious disease prevention protocols to the team, clients, and community.
- Ensures that the work plans align with the short-term and long-term goals of the pilot and provide consistent progress reports.
- Establish and maintain positive relationships with government entities including health departments, clients, co-workers, health care providers, physicians, and the community.
- Continuously expand knowledge and understanding of community resources and the latest COVID 19 and infectious disease information.
- Attends required meetings and participates in committees as requested to support infectious disease management and mitigation efforts.
- Strong and effective written and verbal communication skills, including the ability to prepare and present reports in a clear, concise, and competent manner.
One-year of Public Health experience preferred. Bachelor’s Degree and one year of work experience in a related field or any combination of education, experience, and training which provides the following knowledge, skills and abilities:
- Able to communicate effectively and handle difficult situations with professionalism, compassion and from a Trauma-Informed Care perspective
- Ability to effectively access and utilize Microsoft Office Suite, HMIS, e-mail, database software, and other systems and equipment, and to learn new software as it is required.
- Ability to use a computer, phone, and copy machine.
- Ability to interact in a professional manner with a diverse workforce, clients, and the public.
- Ability to effectively organize work, determine priorities, make independent decisions, and complete assigned tasks.
- Excellent verbal and written communication skills; attention to detail.
- Remains calm and responds appropriately in crises.
- Ability to maintain confidentiality of information.
- Must be able to walk up and down stairs.
- Ability to lift up to 35 lbs
- Must be at least 21 years of age, possess a current driver’s license, have a good driving record and be insurable on the agency liability policy.
- Pass Utah DHS - Office of Licensing background screening for those working with vulnerable populations.
