Description
This Housing Case Manager position will manage the Youth Rapid Rehousing Program which provides housing focused case management services to with the goal of clients quickly transitioning from homelessness to independent and stable living. Additionally, our Rapid Rehousing Case Manager will develop accessible housing resources and community partnerships. You will be based out of the Youth Resource Center; a refuge for youth ages 15-22 experiencing homelessness, mental health conditions, and extreme disconnect from positive adult role models and who benefit greatly from the consistent support of healthy adults.

Volunteers of America, Utah provides a bridge to self-reliance and health for vulnerable individuals and populations who struggle with homelessness, addiction and mental illness in our Wasatch Front communities.


Essential Duties
  • Assist in developing Rapid Rehousing Policies and Procedures with Director of Youth Services: vulnerability assessment protocols; coordination with youth service providers in the community; data tracking and reports as required by funders.
  • Provide case management services to clients.
  • Participate in scheduled clinical case review and prioritization meetings to ensure appropriate case planning and client referrals.
  • Assess client needs utilizing appropriate, community approved assessment tools.
  • Identify priority needs and coordinate services as needed.
  • Utilize agency approved therapeutic Motivational Interviewing techniques.
  • In conjunction with the client, develop treatment, program/service, referral and/or discharge plans to establish documented continuity of care and as a bridge to additional services.
  • Collect all required client information and complete documentation.
Qualifications
A degree in the field of social services and three years of experience working in the social services field or any combination of education, experience, and training which provides the following knowledge, skills and abilities:

  • Protects the safety of all information handled
  • Work is performed inside and outside of the Youth Resource Center; must have ability to climb stairs and enter apartments.
  • Pass a criminal background check and a drug screen.
  • Valid driver’s license and clean driving record (no more than 3 moving violations in the last 3 years, no DUI in the last 5 years).
