Youth Shelter Case Manager 

Schedule: Full time, benefits-eligible! Monday through Friday 8am-5pm but will require flexibility based on client and program needs.
Pay: $20 / hr


Benefits - Approximate full-time benefits package value = $21,000+

Program Mission

To recognize and meet the distinctive needs of youth ages 18-24 who are experiencing homelessness by providing stable housing, mental health care, case management, life skills training, and connection to community services and resources with the ultimate goal of assisting all clients in obtaining and sustaining safe and stable housing.

 

Position Summary

To provide consistent, individualized, and thorough case management services to clients at the Youth Resource Center with the goal of clients obtaining stable housing solutions.

 

Essential Duties

  • Engage with youth to create individualized housing focused case plans to support youths’ desired outcomes. Provide weekly check-ins to all clients on case load and complete case notes for each interaction
  • Utilize the housing first model when working with youth to create sustainable and trauma informed solutions to homelessness
  • Engage with clients to complete intakes to the Youth Resource Center and orient new intakes to shelter services
  • Provide culturally responsive services to youth experiencing homelessness
  • Ability to interact with clients by demonstrating genuine empathy, concern and understanding of clients and their difficulties while maintaining professional boundaries
  • Build and maintain positive relationships with community partners and attend community meetings to advocate for youth driven solutions
  • Ensure that all paperwork related to each case is properly completed, managed, and confidentially stored in the appropriate location.
  • Assist with on-call support on a regular rotation
  • Maintain adherence to client rules and program expectations
  • Regularly attend team meetings and trainings
  • Use agency vehicles to provide safe transportation to clients as necessary for work duties
  • Submit all paperwork in a timely manner and records important notes in shift change log, UHMIS or other documents as appropriate
  • Ensure a safe environment for staff, volunteers, and clients and assesses medical emergencies and provides CPR/First Aid as needed.
  • Exhibit professional boundaries with clients and empower youth towards self-sufficiency
  • Attend work as scheduled and arrive at the designated time.  
    • Work must be performed on-site at the Youth Resource Center due to the need to provide direct client care.
  • Perform other functions as necessary or assigned.


Qualifications

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. High school diploma or equivalent required and 2 years of social services or homelessness-related experience or any combination of education, experience, and training which provides the following knowledge, skills and abilities:

  • Proficiency in Microsoft Office programs
  • Must be able to qualify and pass Utah Department of Human Services Case Management Certification within 60 days of being hired.
  • Ability to utilize Trauma Informed Care and Housing First practices to act and make sound decisions within the scope of position responsibilities as well as reach out for supervision when necessary.
  • Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public.
  • Willingness to accept supervision and direction.
  • If driving is a part of the job, must be at least 21 years of age, possess a current driver’s license, have a good driving record, and be insurable on the agency liability policy.
  • Must be able to pass Utah-DHS – Office of Licensing background screening and pre-employment drug screen.
 

Physical Demands

  • Ability to lift and carry 25 lbs.
  • Ability to move around the facility, interacting with clients for extended periods.
  • Work is generally performed in an office environment.  May entail using a computer for extended periods of time.
