Description
Volunteers of America, Utah provides a bridge to self-reliance and health for vulnerable individuals and populations who struggle with homelessness, addiction and mental illness in our Wasatch Front communities.

Provide outreach case management services to homeless individuals and/or families in Salt Lake County. Objectives will be to create supportive relationships with homeless individuals through street outreach and provide case management support towards individual stabilization and permanent housing.

Essential Duties
  1. Provide intensive case management services to clients, which includes but not limited to connecting clients with housing, benefits, medical, employment, mental health, substance use and other social services as needed. Make referrals to other community resources, when appropriate, to address client needs.
  2. With outreach team members, meet with clients wherever they are that day. This may involve walking to areas where people experiencing homelessness tend to frequent.
  3. Works with clients on obtaining housing through the community triage process as needed.
  4. Participate in regularly scheduled staff meetings, clinical case reviews, and/or interagency case consultations in order to ensure appropriate case planning and referral of clients to the most appropriate service provider.
  5. Assess client needs utilizing the SPDAT vulnerability assessment tool, or other tools as directed, on a regular basis to track client progress and assist in clients accessing emergency shelter and/ or housing.
  6. Assess medical emergencies, contact emergency medical, and utilize Naloxone kits when appropriate. Complete an incident report on all emergency or critical incidents by the end of each shift.
  7. Utilize de-escalation skills to manage client crises as outlined in the agency’s training module on verbal de-escalation.
  8. Ensure that client files and related paperwork are maintained in accordance with licensing and contract requirements.
  9. Appropriately utilize agency funds, manage purchase receipts and submit them in a timely manner.

Bachelor’s degree with either SSW or LSAC or any combination of education, experience, and training which provides the following knowledge, skills and abilities:
One year case management experience
Valid Utah driver’s license with clean driving record.
Works effectively and ethically with a diverse group of people.
Maintains appropriate, professional boundaries with clients and coworkers.
Exhibits clear communications skills and cooperates with others.
Takes action and makes decisions within the scope of position responsibilities.
Considers alternatives and consequences before making independent decisions.
Remains calm and responds appropriately in crises.
Ability to walk in unpaved areas in various types of weather.
Pass background check and drug screen.
