Description

Volunteers of America, Utah provides a bridge to self-reliance and health for vulnerable individuals and populations who struggle with homelessness, addiction and mental illness in our Wasatch Front communities.

The Housing Case Manager will provide consistent, individualized, and thorough case management services to clients with the end goal of transitioning them from the Youth Resource Center to independent and stable living. This position focuses on permanent supportive housing vouchers. You will be based out of the Youth Resource Center; a refuge for youth ages 15-22 experiencing homelessness, mental health conditions, and extreme disconnect from positive adult role models and who benefit greatly from the consistent support of healthy adults. Additionally, part of your time will be spent at the Bud Bailey Apartment Community.

Essential Duties

  • Provide case management services to clients.
  • Participate in regularly scheduled clinical case review in order to ensure appropriate case planning and referral of clients to the most appropriate service provider.
  • Assess client needs utilizing appropriate, agency approved assessment tools.
  • Identify priority needs, (i.e. pregnant, IV/IM drug user, dual diagnosis, and other medical conditions) and coordinate services as needed.
  • Utilize agency approved therapeutic techniques as outlined in the training module Motivational Interviewing.
  • In conjunction with the client, develop treatment, program/service, referral and/or discharge plans, as appropriate, in order to establish documented, continuity of care and as a bridge to additional services.
  • Spend a portion of time working with Property Management at Bud Bailey Apartments where the majority of case load resides.
  • Participate in on-call rotation.
  • Document as required.

Qualifications

  • A degree in the field of social services and three years’ experience working in the social services field or any combination of education, experience, and training which provides the following knowledge, skills and abilities:
  • Must have State of Utah Case Manager Certification or be able to procure certification 90 days preceding hire.
  • Works effectively and ethically with a diverse group of people.
  • Maintains appropriate, professional boundaries with clients and co-workers.
  • Exhibits clear communication skills and cooperates with others.
  • Takes action and makes decisions within the scope of position responsibilities.
  • Considers alternatives and consequences before making independent decisions.
  • Remains calm and responds appropriately in crises.
  • Proactively expands and integrates knowledge of community resources.
  • Protects the safety of all information handled.
$17.00 per hour.
