Make a difference in the lives of adult clients by providing case management services in our residential detox facility. Work in partnership with Intermountain Healthcare and Home Health to meet the needs of clients in the PICC Line program and to coordinate services.

• Meet with PICC Line clients and other clients on a regular basis to set goals and discuss progress.
• Attend weekly client staffing meetings with Intermountain Healthcare and visit clients in the hospital prior to their admission to Detox.
• Create positive working relationships with Intermountain Healthcare, Home Health, and Project Reality. Work together to meet the needs of the clients and arrange appropriate care.
• Develop client referral plans to appropriate treatment facilities and/or other community resources.
• Lead weekly Seeking Safety groups for clients.
• Provide crisis intervention services as needed.
• Identify special needs and coordinate services as needed. 
• Document all case management services provided and participate in case reviews. 
• Learn, utilize and endorse agency selected clinical techniques, including Motivational Interviewing and Trauma Informed practices.
• Participate in on-call schedule.

  Qualifications

• Bachelors degree in the field of social services and two years of work experience in the substance abuse treatment field or an equivalent combination of education and experience. 
• Training and/or understanding in the Trauma Informed Approach, Motivational Interviewing, Trans Theoretical Model - Stages of Change.
• Knowledge of substance abuse, mental illness, and homelessness.
• Maintain good boundaries and confidentiality.
• Computer skills - Microsoft Office
• Ability to walk up and down stairs.
• Must be at least 21 years of age, possess a current driver's license and have a good driving record.
• Must be able to pass a drug screen and criminal background check.

Salary:  $30,160
