Description
Provide individualized supportive case management to clients with the goals of 1) assisting clients to achieve/maintain stability and independence and 2) providing assistance accessing resources to help clients meet their physical and mental health needs. Cornerstone Counseling Center provides services to clients seeking mental health treatment in the areas of substance abuse, mental health, and domestic violence.

Essential Duties
  • Provide service coordination for clients seeking benefits from the Department of Workforce Services, Medicaid, Social Security, HEAT, and other community resources.
  • Initiate client contact and respond to phone messages within 24 hours.
  • Track Medicaid applications and report to supervisor the details of approvals/denials.
  • Complete documentation on the day of service.
  • Communicate client needs/progress with the client’s primary therapist.
  • Perform administrative and clinical support functions to assist the APRNs including: making/ answering phone calls, scheduling appointments, ccommunicating with pharmacies and other providers; entering data on spreadsheets and Order Connect software; and assisting clients regarding their medications.
  • Initiate daily tasks by being proactive, working as a team member and collaborating with other community agencies.
  • Participate in program staff meetings on a regular basis.
  • Assist clients in applying for housing assistance as needed.
  • Maintain positive professional relationships with appropriate community contacts, referral sources, and other professionals.
Qualifications
  • Diploma/GED and work experience with adults with mental illness and substance use disorders, or an equivalent combination of education and experience.
  • Social Service Worker (SSW) preferred.
  • Knowledge/experience working with at-risk populations is preferred.
  • Knowledge/experience collaborating with community resources.
  • Ability to build rapport with clients and engage them in case management.
  • Ability to maintain a calm demeanor amidst potentially high distress population.
  • Energetic, resourceful, problem solver.
  • Basic computer skills including Microsoft Word and Excel spreadsheets.
  • Must be able to pass a criminal background check and drug screen.
  • Valid driver’s license and good driving record.
