Description

Provide consistent individualized case management services to formerly homeless male clients ages 18 - 24 who live in our Young Men's Transition Home. Assist in the supervision of the facility and aid residents in working toward self sufficiency, gaining permanent housing, and increasing income. Full-time M - F days, evenings as needed. .
 

Essential Duties
  • Provide case management services to clients including assisting with housing applications, employment development, life skills, and educational goals.
  • Identify special needs (substance use, dual diagnosis, medical conditions, etc.) and assist staff in coordinating services as needed.
  • Provide crisis prevention, intervention and safety planning services, along with stress management and life skills development to clients.
  • Develop positive relationships with referral resources, community partners and obtain up-to-date information about resources available for clients. Follow-up with clients and agencies to document use/success of referral.
  • Utilize agency approved evidenced based practices as outlined in the training module Motivational Interviewing, Trauma Informed approach, Positive Youth Development Model.
  • Create groups and/or outings with input from youth that will offer them opportunities for personal growth.
  • Participate and present case review during staff meeting in order to ensure appropriate case planning, provide updates and feedback, as well as any other pertinent information regarding the residents and their progress in the program.
  • Document all case management services provided to clients.
  • Contribute to an environment of mutual trust and respect by treating others fairly, demonstrating empathy, and effectively managing interpersonal conflict to preserve constructive, professional relationships.
  • Participate in on-call schedule.
Qualifications

Bachelor’s Degree in social services field or 2-3 years of experience as a case manager and/or working with high-risk youth in a shelter and/or transitional/supportive housing program and or any combination of education, experience, and training which provides the following knowledge, skills and abilities, including but not limited to:
  • Understanding and/or training in:
  • o Positive Youth Development Model
  • o Trauma Informed Approach
  • o Trans theoretical Model - Stages of Change
  • Ability to utilize all the tools provided that will assist in your work (i.e. phone, computer, fax, etc.)
  • Ability to interact in a professional manner with a diverse workforce, clients, and the public.
  • Excellent verbal and written communication skills; attention to detail.
  • Maintain appropriate, professional boundaries with clients, volunteers and co-workers.
  • Remains calm and responds appropriately in crises.
  • Ability to maintain confidentiality of information.
  • Ability to prioritize multiple tasks; flexibility to respond to changes.
  • Organize, prioritize and independently complete multiple tasks.
  • Must be at least 21 years of age
  • Must be able to walk up and down stairs.
  • Pass a criminal background check and drug screen.

$14.50 per hour
